This is how our Cobra works. The employee is notified they are eligible and they can choose to take it or not. The first payment is due within 45 days and must include previous months and current months if they wish to continue. They then must pa…

We allow our employees to cash out all but 1 week of vacation per year. Every employee must take at least one week off for vacation per year. The employee can fill out a cashout request anytime.

Sorry Don, but I have to disagree. I have had several employees that have voluntarily quit to follow their husbands, whether it was for a new job, or the military and they ALWAYS get unemployment. I don't get charged for it though.

May I join the bandwagon please. I also would like it faxed or emailed to me. My fax # 503 289-8739 or email: awallis@nwcontainer.com Thanks x:-)

I don't know if this helps, but our payroll is done through PrimePay and they charge $6 per pay day for direct deposits whether we have 80-150 people.

We are a company of about 100 employees. We have 2 different insurance choices and 3 different types of coverage. The base plan (HMO) is $25.00 for employee the buy-up plan is $45.00 for employee base plan for employee + child(ren) or employee +…

Any kind of paperwork that I want to have proof of sending I use certificate of mailing. Alot of people won't accept certified mail, as it usually means "bad news". This way I have proof that I sent the information (and its cheaper than certified)…

Our policy is that all incidents/accidents must be reported within 24 hours, no matter how small or trivial the incident/accident seems. All we require is that at least the in house form is completed, so that if in the future there is a problem we …

I agree also with Don. If the employee had wanted his/her supervisor to know the details he/she would have told them. I speak from experience, when I was going to go out on MLA to have my leg amputated I really didn't want anyone else to know, bec…

Yes, I believe you should offer her the w/c paperwork. The w/c will determine if it is work related, it could very well be. W/c isn't only for accidents it can be from an activity that over time causes problems, such as carpal tunnel can happen fo…

We have an in house incident/accident report that we use for this type of thing. We don't send anything to WC at this time. Later if the employee needs to see a Doctor then I will send a copy of the incident/accident report along with the 801.

You said this person is an "employee", that as far as I know means that you are responsible for their workers comp. whether they work 1 hour 1000 hours or more, and whether they make minimum wage or higher. The only way I know that you wouldn't have…

Yes, my understanding of the FMLA is that it is to be treated as if the employee was working during that time. Any accrual for PTO or Holiday would continue as if they were at work.
-
Our company policy states that you stop accruing vacation after 30 days of absence, other than FMLA. The employee is allowed to use any time they have on the books, but will not accrue anymore until they return to work. If they are gone less than …

I agree with Don th-up

I agree 100% with Don D. I think not terminating the employee now is the right thing to do. Give them what little hope they may have of having a reason to fight to live.
-
Here's my thoughts on this: I would tell the employee that they always have the right to make a Dr. appt. but that they should schedule it before or after work or on a non-work day. If they don't and it's not FMLA related they will fall under the …

We had an employee recently that this happened to, we just gave her a medical leave of absenc. She was only gone a couple of weeks, and wanted to get back to work to help ease the pain. It worked well for her and us.

I agree that an individual on FMLA whether paid or not, must continue accruing benefits. My understanding is that when an individual is on FMLA they are treated as if they are working.

I'm not sure if this is state specific but in Oregon "Power-driven hoisting apparatus and lifts are prohibited under 18 = H.O.#7" I took this directly from the Certificate to employee minors that must be posted in each location where minors are wor…

99% of our donations are for a major reason, either the employee themselves need the time off and won't have enough vacation to cover them, or someone in their immediate family is hospitalized and the employee would not be able to be with them witho…

We do allow employees to donate vacation days or their personal day to another employee, the employee donating fills out the paperwork for the amount of hours and to whom they are donating. Then the hours of donation are deducted from the the donor …

We have an unwritten policy also. We always send flowers for the funeral of immediate family members, an employee who is hospitalized, an employees spouse, or child if they are hospitalized for surgery or a serious matter. We also will send flower…

I have had this happen only once in the 20+ years I have been in Personnel. The employee was told we had accepted their resignation and that if they wanted to reapply when we had a job opening they could. x0:)

I have always been lead to believe that as a payroll administrator it was against the law to tell anyone what to claim on their w4, because you are not their tax advisor and should leave it completely up to them. I have employees who claim exempt a…

Don, You have a wonderful heart and I thank you for it.

Don, I won't stone you, and I'm not going to agree with the anal, x:o but to each his/her own. Do you just keep adding I9's to your pack forever? Doesn't it get rather large and hard to handle x:-/ If you have a very large turnover, or lots of n…

I have 2 I9 books. One is for current employees and the other is for termed employees. When I pull the Personnel file for a termed employee I also pull the I9 and write the term date in red on top and then file it at the back of the I9 term book. …
