Computer Use Acknowlegement and Consent form

We are getting to the point where we are spread out in three locations and loosing control over the use of the computers - primarily downloading of programs without getting our Newtwork Administrator's approval and help so that it works with his programs.

We want to advised people not to do any downloading without checking with him first and that doing so will lead first to the administrator deleting the program, secondly the administrator putting a block on an individuals computer and lastly coud lead to terminiation.

We want to be firm but not overbearing (if that is possible) and we want a signature block that says the enployee has read and understands the consequences.

Any ideas?

    They have a slew of IT related sample policies. The Acceptable Use policy might be along the lines of what you are looking for since it is more all encompassing. If nothing else it might be a good starting point, although you would want to add what the penalties are for violating the policy.

    1.) Removal of material, etc. from PC on first offense
    2.) Lock out from PC for second offense
    3.) Termination of employment for thrid offense.

    I would suggest having each employee sign and date the policy and put it in their personnel folder. With those 2 new viruses that just recently went around (Klez and Bugbear, we were hit by both!), network and PC security should be a main concern.

    The Klez got into our payroll server and wiped out several files. Luckily IT was able to restore them and nothing else was damaged beyond control, but it could very easily have resulted in no paycheck for employees (or at least that is what we told them all!)
