Exit Interviews

I am looking for a form to use for terminations. Ideally, the form would have space for employee's statement of voluntary quit, plus room for managerial comments regarding the employee.

In the future, when called for a work reference on a former employee, we would then just pull out this form and see (1) employee's reason for leaving, and (2) managerial comments about the employee.

Does anyone have a sample form to share? Any advice on what to include/not include on this form?

We use a Personnel Action Form for terminations, which has the reason for termination as well as a "Comments" section for management to write any additional notes they deem necessary, as long as it pertains to the reason they left. This form is also used for vacation time, sick time, jury duty, floating holidays, raises, new hires, etc. - Kind of a "catchall" type form.

For the employee who is leaving, we have an Exit Interview. This is a form where they basically have the opportunity to provide us feedback on their experience working with us. On the back of this form is a checklist of a variety of items that you initial and then have the team member initial signifying that they agree with the item.

We have two separate forms. One is the exit interview where the employee answers questions about the good and the bad of their jobs/the company/their supervisor, etc. The exit interview form is kept separate from the personnel file. The exit interview is to be used as a tool to track why people are leaving and also to evaluate if there is a problem in a certain department, etc. Their exit interview should not be held against them (we are hoping for an open and honest response, right?), so we don't file them in the personnel file.

The other form is the termination evaluation which the supervisor evaluates the employee briefly (not a full-blown evaluation)...this is where we document if the employee is re-hirable and if the company property was returned.

I'd be careful about sharing managerial comments, unless you get a signed release from the employee. The comments might be useful, for example if the employee reapplies. But if the comments are bad, giving them out could put the company on the hook for a defamation lawsuit (especially if there is no guidance given to managment about what goes in the comments, and later on you can't even tell who made them).
