Notice of Pay Rate and Payday

Does anyone have an MS Word version of NYS Notice and Acknowledgement of Pay Rate and Payday that is required to be distributed to employees by February 1st?  I see the template on the DOL website, however it is in a PDF format and I'd like to modify it. I have searched the web for a MS Word version but have not found anything.  Has anyone else come across one?
