Company Smart Phone Usage Policy and agreement

Hello, I am updating our cell phone policy to include the Usage of Smart Phones. As you know, the usage of these phones increase potential of a serious security threat to the company network and company data stored on these phones. I have a lot of information regarding what the contents of a Company Smart Phone Policy should include. What I am looking for is an actual written policy. Does someone have a written Smart Phone Policy they could share?
