Has anyone used surveymonkey.com, or other commercial web-based self-design survey tool, to collect eeo self-ID information from applicants? Can you think of any reason not to use such an instrument for self-ID data collection?

I've used online survey tools to collect self id information very successfully in the past. My favorite online survey tool at the moment is http://www.websurveymaster.com/ It allows the collection of all sorts of demographic information. It is very easy to use, has great result charts, and for this type of self id information it is extremely easy to export to Excel. Best of all it is free for small to medium size surveys.
