MO letter of Dismissal

I have never done a Letter of Dismissal/Termination based on MO requirements (our home office is in NE), and I was hoping someone may have a template for a Letter of Dismissal that is required by Missouri law when an employee requests one.  If anyone has one they are willing to share as a source of guidance, it would be much appreciated.  I am not sure how specific they typically are for reason of termination, i.e.  unacceptable and unprofessional behavior toward fellow employee or be more specific?

Yes, I am a member and checked for a sample form but did not find one.  I did find the general description of what should be included, but was just uncertain as to how much detail to include in regard to the facts leading to termination.  I know have I the basics, but was just wondering if anyone else that is a member had actually done and issued one of these and had some pointers.
