NJ FLA Clarification

I have two questions pertaining to NJ's Family Leave Act that I was hoping someone could help me with.
First, in regards to the birth of a child, an ee is entitled to 12 weeks of leave to care for the child as long as the leave begins within one year of the DOB. Does that mean that the ee must use that 12 weeks within the first year from the DOB or can they use the 12 weeks over the 24 month period as long as the first day of leave is within the first year of the DOB?
Second, I have searched the internet and cannot find any actual forms that an ee must complete in order to utilize NJ FLA. Is anyone aware of any state specific forms for the NJ FLA?

I too have never seen forms for NJFLA. I believe that if the law states w/in one year of the DOB then it must be in that year bnnot the 24 mos. period.

I've only seen it state that the leave must begin within a year of the DOB on the state site. But I recall reading on BLR that the 12 weeks of leave for a child must be taken within that first year after the DOB even though NJ's leave operates on a 24 month time frame. I'm (more than) a little confused. And NJ's paid leave hasn't even started yet! 3 forms of leave to deal with next year...ugh[:'(]
