personal cell phone usage and texting

Can anyone share with me a policy which prohibits texting while operating equipment or driving?  Also, anything that you have limiting personal cell phone usage would be appreciated as well.  We have worksites where employees are operating heavy equipment while they are texting or chatting on cell phones.  Thanks

No employee is to engage in the use of a mobile phone,(texting, talking, listening, dialing, etc.) while operating company equipment or a company motor vehicle while the vehicle or heavy equipment is in motion.  The only exception to this policy is where a phone call is made in a bona fide emergency, such as to call “911” or a similar emergency number (e.g., to call an ambulance, fire department, etc.)<?xml:namespace prefix = o ns = "urn:schemas-microsoft-com:office:office" />
