Does anyone know about the OSHA 300 log being revised? I see the rev 1/2004 is still on their website but an insurance company emailed one saying the forms have been revised??? If they have been revised does anyone know if the 1/2004 revision is still acceptable?

Federal OSHA has not revised the 300 log. If you are in a state-plan state that uses different forms, check your state.
