How to maintain personnel records

I was given the assignment to update our filing system. What records does federal and state laws require we keep and can we file everything together?

The following items should be kept in files separate from personnel records:

I-9's
Anything with medical information
EEO/Affirmative Action identifying information

I would, but then I'd have to ask PICB how to add an attachment to a post and I don't want him to think he knows more than I do.;;) I know how to do a link and a picture, but not an attachment.
