Multi-Rater (360 degree) assessments

I am interested in implementing a 360 degree feedback process in our organization as we have several tiers of managers and I think it could provide some useful development and training information. Any opinions out there? Anyone using this process successfully? I would love to see a sample of a feedback tool if possible. Thanks!
