Employee status change form

Good morning!

Does anyone have a form they use to inform HR and payroll of an employee that goes from FT to PT or Vice Versa.

I just e-mailed a form to you. Forgot to tell you that we used to use four-part paper to print it on, so that one copy went to payroll, one to HR, one to supervisor, and one to employee. Now, I print it on regular paper and copy if needed - there is only one file (HR), payroll and supervisors don't keep files here.
