Leave of Absence

We are looking into creating a 'Personal Leave of Absence' policy. Does anyone have any samples they would like to share? I have the one that was listed on this website.

  • We allow 4 weeks of unpaid personal leave in a "rolling" 12 month period provided that:

    The EE does not qualify for any other leave type such as FMLA, Breavement....

    and

    That the EE is not on any type of written disciplinary warning.

    We do also require that the leave may only be taken in increments of 5 consecutive working days so we do not have many takers that can afford to be without a check for a week. We put this in place primarily to assist the first year employee that had medical concerns prior to being eligible for FMLA.
