Check in list of items received

I am in the process of creating a form that new hires have to sign a form of the items that they receive when they are hired in. Example of some of these items would be
keys
swipe card
parking card
employee hand book (we also have an employee manual handbook acknowledgement)
I would use this form/check list for when the employee leave(during the exit interview).

My questions are:
Does anyone use a form or check list like this? If so, do you charge for certain items that are not turned in? Do you mind sharing this form with me?
