Documentation forms for supervisors

HR in MN

Hi, I am an HR director for a small state agency in Minnesota. In one of the last HR Hero newsletters, it recommended that HR offices develop a documentation form for supervisors to use to make sure they are documenting issues immediately and accurately.

Before I reinvent the wheel, I was wondering if anyone out there has already developed a form like this..? If so, would you be willing to share? I want to make sure I am not missing any important elements.

There are some basic forms in the Subscribers Area of this website, if you subscribe to Minnesota Employment Law Letter.

Once you log in with your password, click on "Forms" (under HR Tools in the middle of the page). Then click on "General HR."

Thank you! But I am having trouble getting to where you recommend. I login (I do subscribe)and in the middle of the page is HR Tools. There is no category for "forms" there. It is just: hot new tools;employment law;HR products by state and Train your supervisors. Hence, no chance to get to "general HR". Help!

Go to top Right corner of for Subcribers Login
Once logged in go down the page to HR Tools
underneath there is Forms, and two other topics.
