Evaluation by direct reports

We are promoting the term partnering and in doing so would like to have direct reports do an evaluation of their managers. We want the evaluation questions to be directed at strengthening the partnership. We may even asks some of our suppliers to evaluate the partnership between them and our purchasing manager.

Does anyone have a form that might fit this bill? Anything at this point would help.
