We want to pay one of our psychologists a bonus rate for every additional psych eval (past 5) he does in a week's time. We'd like to include whatever bonus money he makes in his bi-weekly paycheck. Does anyone see anything wrong with that? Thanks for the input.

What concerns me, based on what you have written, is that you are talking about one individual. That could be problem. The guidelines should apply to all the psychologists who do evaluations. Have you thought about a merit increase?

He is the only person on staff who does this kind of work. The other folks who generate evals of this type are all contractors.

Since this psychologist is the only one in this category, it should not present a problem and you will be paying it as income. However, do you plan to just give him the bonus each week/month, or will it be tied to timely submission of his client contact forms and/or paid only if the billings are reimburseable?

Only if reimbursable, but he wouldn't get a case to evaluate unless it was pre-approved for reimbursement.

You can really structure a bonus program any way you wish. Since he is the only employee who performs this function, there shouldn't be any issues with fair and consistent administration of bonuses. We have often used bonuses as a way of rewarding individuals without overly inflating their annual or hourly salaries.
