Employee Satisfaction Survey

I'm working on developing an employee satisfaction survey or a workplace environment survey and was wondering if anyone had suggestions as to good questions to add to the survey. I'm looking at both the physical aspect of our work environment as well as the emotional aspect. If you have any good websites I could look at for ideas or potential surveys, that would be great as well. Thanks -
