Salary continuance/short term disability

What are employers offering in terms of salary continuance/short-term disability? What are the eligibility criteria? If you provide paid leave after a certain period of employment for sick leave, do you allow employees to take "unpaid" sick leave prior to being eligible for paid sick leave? Do you continue benefits? In lieu of leave, if they are sick during the eligibility peior but not eligible for paid sick leave, do you terminate employment? (This is in addition to PAL time which includes sick time and assumes there are is no ADA question or FMLA--i.e., new employee).
