FMLA & Rearranging Work Schedule

Here I go second guessing myself.
Employee's schedule involves working every other weekend. Has medical documentation that she needs to provide 24 hour supervision to her daughter and due to family scheduling of providing care she can not work the weekend anymore.
Instead of giving the weekend off (unpaid and counting as intermittent FMLA) - we offered and she accepted to a change of her hours to M-F only.
It's been explained that it is a temporary situation while her daughter needs this supervision but since we are not counting this as FMLA (because she is still working her 40 hours now) what documentation can we request say 90 days down the road to ensure the supervision is still required?
