Someone requested to extend their FMLA leave beyond the time certified. Would a doctor's note suffice for you or would you ask for the info on a certification form?
It would totally depend on the wording of the note. I've gotten notes that are suspect, such as "Treated on 8/12, followup appt on 9/12, possible return to work on 10/12." I would require a complete recertification in such a case.
