ADA Accomodations Review Process/Procedure

I used to work for an employer who had a very well documented Accomodations Review Process. It defined the Accomodations Review Team Members (by title/function) and the steps each was to take in the review process. We used this process very successfully to review and respond to employee and applicant requests for accomodations.

Unfortunately I have since left this employer, and did not manage to take a copy of this process/procedure along with me. Is there anyone out there that has a written process or procedure they would be willing to share with me-I am working in HR specifically in Manufacturing, so it might help if you are a manufacutering firm (my former employer was a petro-chemical refiner/marketer.)
