OSHA Reporting

How do I report days away from work on OSHA 200 in the following case?
An employee reported on 12/11/01 that he injured his shoulder on 7/1/01 (Yes, that is bad, but not my immediate problem). He had not missed any work but he had received cortisone shots in Sept.
On 12/14/01, the same employee reported a knee injury which occured on 12/13/01.
The employee is now off work, probably because of the knee but even if the knee was well the shoulder is now bad enough he still couldn't work. Do I report the days off under one injury and leave the other blank? If yes, which injury?
