FMLA - Off for oneself -vs- off work for family member

In need of your feedback. I just learned that we (the company) only send out the "FMLA letter package" to those employees who are going to be off work while caring for family member, etc. We do not send out the FMLA letter package to employees who are taking time off for back operations, hysterectomies, pregnancy, etc. Is this the norm?!?!?!? I had just assumed that the letter went out for any employee taking time off from work for a disability for themselves or situation involving family member. What say you forumites?

Are you counting the "self" leave as FMLA?
