FMLA Forms

My company uses the DOL's Certification of Health Care Provider form in order to certify FMLA. Can we insist that all employees have this completed for FMLA purposes (as opposed to using other information)? We have employees turning in all different types of information and we'd like to standardize the process.

Certainly...... If you use the DOL forms, insist that they be used consistently.
