Vacation, Sick/Personal Time Accruals and FMLA

I am interested in receiving a few FMLA policies. Specifically, as they relate to accruals while on paid/unpaid FMLA. I understand that in many organizations there is no accrual of vacation/sick/personal while on unpaid FMLA and that the policy must treat any unpaid leave the same as an unpaid FMLA. 5-7 samples would be sufficient.
