MSDS verification form

Our company has now completed a new MSDS program and we are looking to retrain all of our employees. Once they have completed this training we would like them to sign a form stating that they have completed this training course and they are now trained in how to handle any situation involving hazardous chemicals. Does anyone have a certification form for MSDS training that they would be willing to share?
