Can we ask for the current employee's name and position? I've stated that reported data will not identify any specific individual.

You kind of HAVE to have them give their name on your self-identify form or you won't know if you've gotten everyone's info.

The reported data won't include names, but you need it for your own tracking purposes.

I mail-merged names and location #'s (we are multi-site) onto the self-identify form before distributing, so all the employee has to do is mark some boxes.
