Accident Report

Does anyone have an internal accident report they are willing to share. I just had a visit from our Work Comp's Safety Rep and he said we need a new one. Work Comp Report, Vehicle, I'll take whatever anyone is willing to share. I haven't been on in a long time - but I hired some help and able to catch-up with work and also with the forum.

I emailed you one I made that we use for all types of accidents (I not only handle workers comp but also all our property/casualty insurance for the whole company. Hope it helps. Jill
