Hello everyone! I need some quick advice...I just updated our employee hndbk for the first time since it was first completed and released in June. I am going to communicate the changes out the staff, so I wrote a memo that I will personally handout with the list of items that have been changed.
Along with the memo, I would like to attach a form that requires their signature. This form will state that they have read, understood and had the opportunity to ask questions on the changes. Is there anything else I should state on the form? And yes, the form will go in their files.

I don't include the words "I have read and understand". That causes people to balk. We just have a general acknowledgement of the handbook or whatever it is and clearly identify the issued material by title, date of publication or revision and a brief explanation of what it is they are signing for. You'd be surprised (maybe not) in the UI hearings how many people will say 'Naw, that ain't what I signed for. That was something else. It wasn't no handbook. It don't say so on here.' The beginning also states that, "By my signature below, I acknowledge that I have received the XXXXXXX revision of the company X employee handbook."
