Hello everyone!
I am attempting to create a performance appraisal that employees can use to evaluate their direct supervisor anonymously. Does anyone have any advice on what the criterion should be for management level?

My personal opinion is that I do not put much faith in a document that the author refuses to sign.
I think I will get a lot more information that way and also give employees the opportunity to evaluate management.
I'm emailing you a simple four-question form that our Board asks the Superintendent's (I'm at a school) direct reports to complete each year as input to their annual performance appraisal of him. Hope you find it useful.
