I googled it but can't find any actual templates or anything. Perhaps you could make a list of the warning signs of ee violence or signs of stress and make up your own questions around those. Then you could "score" the survey depending on the answers chosen. Of course, it's not like it would be major rocket science or anything but it might help if you're just trying to get an idea of where your ee's stand on these issues. I wouldn't make any of the questions too "easy" or obvious. Try to make them have "no wrong answers".
