Background Checks

Could someone help me out with a sample acknowledgement and authorization statement that we could include with our employment applications for background checks? This would need to comply with California law, since we have the luck to be a small business operating in the "cutting edge" state.

If you are outsourcing these backgrounds, that company would have the required release verbage not only for them but to also protect you.
