I-9

We just completed an audit of our I-9 forms and discovered that an employee who was hired in 1988 does not have an I-9 form. We don't know what happened to it since there has been personnel changes in the HR Department since then. Our only assumption is because this employee has had 3 different last names since her employment began due to marriage/divorce, that it was mistakenly shredded.

I am not sure if we need to do anything to remedy this. Any suggestions?
