Has anyone conducted an operations audit to see how areas like human resources, finance, general operations were viewed by their staff and how they could improve?

I am looking for somewhat of a satisfaction survey to distribute to our employees (100) to see what we are missing (i.e. being able to schedule conference rooms from desktop) and what they think of our services (i.e. facilities).
