Company property - employee acknowledgement

Does anyone have a sample form for employees to sign re: distribution and return of company equipment? I'm specifically looking to include a acknowledgement statement and authorization by the employee that the value of equipment can be withheld from their final paycheck if damaged/not returned (I'm in Arizona)?
