FMLA

I need a form letter to send to employees when they have requested FMLA, either
granting or rejecting the request. Can anyone help me with that? Thanks!

  • If you will call me with your e-mail address (so you don't have to post it on the Forum), I developed a form for a client that is a combination. The top half the employee fills out requesting FMLA and the bottem half the employer fills out to grant or deny the leave. Very simple for the employer. I'd rather e-mail as you can take them right off the e-mail and print your company name in the space I have provided to personalize. No re-typing!

    Do you have a medical certification and a return to work form as well? If not I'll send you those. Hope it helps!
