Simplifying Performance Evals

Our healthcare organization uses job descriptions as performance evaluation tools and they seem to grow bulkier each year as another regulating body adds another requirement. Managers sometimes are late doing evals because it has become a daunting process.
Have any of you found or developed a tool to evaluate employees that is short, effective, and meets healthcare regulations?
