Applications and Debarrment statement

Can anyone tell me if Federal Contractors are required to have the following clause on applications: "I certify that I am not listed as debarred, suspended, or otherwise ineligible for the Federal programs as set forth in the list of 'Parties Excluded From Federal Procurement Programs Publication.'

I thought I had heard that this was no longer a requirement... Can anyone clarify?

I don't know if it's a requirement that the contractor provide the statement, but we require all contractors to sign an agreement that the company has not been disciplined or debarred on either the state or federal level.
