Medical Leave Policies (Not qualified for FMLA)

Hello all, do any of you have a leave of absence policy for employees who would not yet qualify for FMLA that you wouldn't mind sharing with me or know of a resource where I could find one? I am currently trying to re-strucure our policy and am not sure where to start. Thanks so much.

Red, we dumped our personal leave policy and cover everything under FMLA and other forms of required leave in conjunction with our generous no-fault attendance policy. We term and rehire people who need leave outside of that regimen.
If that does not work in your environment, the personal leave policy becomes the classic problem of keeping it open to management discretion without it becoming an unevenly applied benefit. What are the qualifying reasons? Who decides? Etc. I have seen people give non-FMLA leave for FMLA-like reasons, which is an easy way to do it.
