Failure to return from workplace injury

A 2nd shift employee had a muscle strain and went to ER. They put him off for two days. HR tried to contact him repeatedly and he did not answer the calls.

On the date he was to return to work,(4 days later) he did call stating he did not have good reception at home and just saw the mesages and missed calls. He questioned why ER did not do an MRI and said he shoulder still hurt.

He was told to report to work and he would be taken for further treatment. The clinic confirmed it was a strained muscle. The clinic gave him light duty and stated to report in one week if not better.

After the week passed he did not report back to work. Will not answer our phone calls.

Can he be termed? Should a letter be sent? What should be in the letter?
