Communication WC and FMLA are running concurrent

Does anyone have any sample letters of your communication to employees off on WC that FMLA will be running concurrent with it? I've looked all over on-line and can't find one. I want a good way to word this to make sure it's clear, precise and legal. Thanks!

We simply send the WH382 employer response along with a letter explaining that FMLA runs concurrent with their absense due to their WC injury/illness.
