Performance Appraisals and merit increases

I would like to ask and see if anyone would be willing to provide me with a sample of their salary advancement policy or a good site to reference. I work for a engineering firm and we are in the process of changing a lot of our old ways to new ways that will hopefully better our company in the long-run.

Management would like to go from giving out yearly COL raises to merit raises based on the previous years performance. I was asked to provide management with some examples. I have looked at SHRM, but I am still in need of any assistance available.

Also, we would like to change our performance appraisal forms too. Could anyone provide me with information on using appraisals that instead of checking superior, average, ect you give the employee a number for each catergory and then average the numbers to give them their overall average performance grade?

I know that this is a lot to ask and I would like to say thanks in advance for any information provided.
