reason for not hiring applicant

We have each person we interview fill out an application. If we do not hire that person, we don't keep any type of documentation as to why the person was not hired. What is your normal procedure? Do you have a form you utilize for noting why an applicant/interviewee wasn't hired? If so, could you share that with me at [email]jturner@tgic.com[/email].
