I am setting into policy a yearly review for our hourly employees upon their anniverary date... Does anyone have a nice short basic form?

We are a small concrete company if that is any help.

Our new one for next year will be 2 pages (we are putting/getting final touches and approvals for it now). If no one else comes forward for you, let me know and I can send you our final 2 page version in about a month.
You will need to give some thought to core compentencies that you want on your form..things every ee, no matter their job, should be reviewed on. You can also find some pretty good generic one's on-line.
