Formal warnings

Hi, we would like to create a formal warning and would like to see some samples of other people's if possible.

Welcome to the forum. What are you considering a "Formal" warning? We treat any warning the same. Some may be verbal and others written but a warning is a warning and any and all warnings end with the verbiage that any future violations may lead to more severe discipline up to and including dismissal. Don't know how much more formal you can get.

sorry, I see your point. We are looking to create a standard form to document disciplinary action and place in the employee's file.
