I just recently realized that our AA Questionaire is not exactly correct. We have extra fields that we shouldn't have and I was wondering if anyone can send me one so that I could use it as a reference guide to how they should look like.

Do you mean the invitation to self identify for applicants or the VETS/Disability for Newly hired employees?

Yes, the questionaire that is voluntary and askes them their race, vet status etc. upon hiring.
