As of January 1, 2002 Illinois employers are required to include a provision on their applications stating an applicant is not obligated to disclose sealed or expunged records of conviction or arrest (IL Public Act 093-0211). Does anyone have more information or legal wording their including on their applications? Thanks.

but leave your questions on the app soliciting the information. Maybe add, "Any omission or untrue statements not in accordance with IL Public Act 093-0211, will be grounds for rejection or termination."
But, I'm no attorney. Run a decision like this by one. The design of an application form is important enough to always get legal opinion.
