Authorization for section 125 deduction

Does anyone know of a website where I can obtain a sample letter or list of required information for authorization of withholding under a section 125 plan?

Are you looking for a general Premium Only Plan authorization, or something specific to a flexible spending account (childcare and unreimbursed medical)? I can send you our POP, if you'd like. Actually, now that I think about it, the whole Plan is like eight pages long and I don't know that I have it all in electronic format. I do have the single-page authorization form, though.

We are looking for authorization for our health and dental plans only. If you have anything or know of somewhere I can get a sample letter from I would appreciate it.
